マルファン症例には最適。

2時間手術。もちろんオフポンプ。冠動脈部は穴が開いている。

ロータスは非常にいい製品。ラピッドペーシング不要。回収可能、再移植可能。PVLも非常に少ない。

コミッシャーを釣り上げて計測することが大切。
手術時間、弁尖に対するWall stress、手技の複雑さは全てReimplantation > Remodeling
Wall stressの強さは遠隔期に影響するかも。
再手術率は年々低下しており、昔のデータで比較するのは意味がない。

BIMAを使う人は、いいに決まっているという思い込みバイアスがあるのではないのか。
各種スタディで生存率に差がでるのは、相当大きなもの。しかしBIMAvsSIMAでは必ず差が出ている。
